1-1 1999年6月1日，美国航空1420

1）降落后停不下来，冲出跑道——驾驶员在飞机降落时忘了将扰流板启动

2）在当年的民航管理规范中，有要求“务必到站”，在此要求下，本该停飞的暴雨天气，航班强制起飞，导致灾难；且民航管理规范中仅明确“不能飞入雷暴”，但并没有明确在什么指数下不能做什么，应该做什么，缺少明确、精准的动作指引。

1-2 1989年2月24日，美国联合航空811航班

1）波音747的舱门固定器设计有问题

2）航运管理当局同时扮演着两个角色，既要发展航空业，又要监督飞行安全，当两者无法两全其美同时兼顾时，多会为了经济效益放弃安全，因为经济是一眼望得见的，而安全可以凭侥幸。

1-3 2001年8月24日，加拿大多伦多 越洋航空236航班

1）在油箱漏油严重时，机长盲目自信，认为油箱不可能有问题

2）右引擎油管爆裂，因为维修过程中操作不当，在执飞前5天已知配件有问题，但航空公司仍强制执飞

1-4 1998年9月2日，瑞士航空111航班

1）MD-11机舱起火，由头等舱的娱乐系统使用过程中系统过热，引起火灾

2）机上材料的耐火标准不够，在火势一起便迅速蔓延

1-5 1996年10月2日，秘鲁航空603航班（波音757）

1）在起飞前清洁工清理机舱卫生时，按要求将静压孔用贴纸贴上后再进行打扫，但事后忘记将贴纸撕下

2）应该验收的督查人员失职，请了普通技师代班

3）且因为贴纸颜色与机舱内颜色相近，导致正副驾驶再起飞前的例行检查时也未能发现

4）所有原因导致飞行过程中正驾驶的静压孔被堵，在仪表盘传送高度数据时出现错误，引起坠机

5）波音公司应承担预知产品误用时可能产生的问题，以及是否有修正空间

6）在此次坠机事故中，清洁工人因过失致死罪名入狱，令一个对后果严重性知道最少、最无辜的人，承担了责任。

1-6 2001年1月31日，阿拉斯加航空261航班

事故发生时，飞机垂直坠落

1）润滑不足导致伸缩螺杆故障，事后检查维修工作程序时，发现工作记录作假，在记录本上记录的是未做过的工作

2）伸缩螺杆在飞机例行检查规范的要求中，检查的间隔时间很长（多年一检），本该换掉的伸缩螺杆，在当任主管检查时被记录在案，但却被下一任主管驳回并删除，为了节省维修成本

3）设计缺陷：没有失效的安全设计

记录一下重点。S1-1:MD80，1、负责控制水平安定面的螺杆润滑油不足；2、零件违规延寿；3、存在单点故障，重要部件没有多余度设计。S1-2：747，货仓舱门固定器强度不够以及C型栓电路设计缺陷。所以说全电飞机的短路保护太重要了。S1-3：757，简单说是胶布粘住静压孔造成空速表和高度表失灵，但是背后隐藏了很多问题。1、飞行员飞行前目视检查不彻底；2、维修或养护时造成状态变动的警示不够，起码胶布应该用红色或者橙色；3、飞行员水平不足，飞行员应该具备判断数据来源的能力。比如气压高度和空速来源于动静压系统（空速管），而近地警告明显和气压高度不是使用的同样的高度，近地警告应该使用的是无线电高度，飞行员应该清楚此时无线电相关告警应该较为可信。同样让我很疑惑的是，难道757机上仪表不显示无线电高度么，如果没有那完全就是设计上的重大问题，如果有，那就完全是飞行员的水平问题。因为无线电高度和动静压半毛钱关系都没有。只要有无线电高度完全可以保证飞行，虽然无线电高度对于使用的高度有限制，能用的范围较小，但是完全可以避免因高度判断失误而坠海的发生。4、空管提供错误的高度信息。雷达系统无法判断飞机的飞行高度，只能通过飞机发给他的信号来获得相关信息，这个感觉不太合理，不是相关专业不太好做评论。但是这样做只会造成用两个相同的错误信息来相互印证的局面，设计逻辑问题很大。同样，和上一条所说的相同，空管水平也有限。在空速出现问题的时候，高度也大概率会出现问题。5、飞行员特情处置能力太差。在速度高度全部失效的情况下，飞机的姿态信号是完全可用的。仅凭飞机的姿态信号保持飞机的稳定飞行是飞行员应该具备的基本能力，在高度无法确定的情况下盲目返航降落有些冒失。在保持稳定飞行的情况下等待其他航班前来救援导引是最正确的选择。表面上是维护人员的一个小疏忽，其实背后隐藏了很多隐患。要成为一个优秀的飞行员需要对飞机有全方位的了解，不能仅仅是知道如何驾驶。S1-4:A320，著名的萨利机长哈得孙河迫降全员生还，鸟击目前在发动机技术上还是无解，只能在控制机场附近鸟类群落和雷达探测上下功夫了。S1-5:MD11，又是一起加改装引起的事故，以贪婪著称的麦道消失在历史长河中不是没有道理的。在加入新娱乐设备的同时竟然连最简单的用电过载保护都没有，不说断路器电源保险盒，哪怕加个保险丝呢。并且与客舱的其他用电没有接入同一个汇流条，也没有设置单独的断电开关，在手册中没有明确新加设备的相关内容，使飞行员陷入了误区。在电路附近的绝缘材料使用的是易燃材料也是匪夷所思。虽说航司的故障检查程序太过冗长，飞行员麻痹大意等也是造成此次事故的原因，但是究其根本这是一次彻头彻尾的低级设计失误。果然，严格的适航条款是制定给国外公司的，美国自己的飞机过适航从来都是看脸和看钱。S1-6:737-800，无线电高度表故障，进而引发自动油门进入减速拉平模式，使发动机进入慢车状态，不巧的是此时飞机正好进入下滑道，飞行员同时操纵减速，自动减速加人工减速叠加，飞行员因高度表告警频繁放松了警惕，发现失速再想拉起已经来不及了。锅可以分成三个，一是波音的无线电高度表和自动油门系统，二是飞行员降落阶段不观察空速，三是地面引导转弯降高度太晚了。但是最主要的原因还是波音的设计缺陷，一个故障率这么高的设备是怎么定型的，怎么过适航的自己心里清楚。还有失效概率这么高的设备连最简单的自监控和比较监控都没有，飞控系统采用数据的时候还只采单通道的，成本真是省到家了。就这样还要把主责赖到飞行员身上，飞机的设计就是要考虑到各种极端情况，作为安全性要求更高的民机不应该更遵循这种标准么。S1-7:加拿大越洋航空236号-2001年-空客a330，在大西洋上空右侧油箱破裂导致燃油泄漏耗尽，飞行员在双发停车完全无动力的情况下依靠滑翔将客机飞回最近的亚速群岛空军基地。事故原因是地勤在更换右侧机翼上的液压管时没有用符合规定的型号，而是用一根看起来差不多的管子替代，最后由于振动原因输油管和液压管不断摩擦导致破裂。启示如下：1、输油、液压、电缆一定要做好抗磨处理，还要保持一定的间隙。有漏油风险的管路要与其他电缆保持一定距离，防止渗油时会对电缆造成腐蚀。2、选用零件时一定要符合标准，否则几毫米的安装误差都可能造成不可挽回的后果。3、对飞行员的告警培训一定要全面，处置程序一定要清晰明了。油温低油压高等告警产生的原因要向飞行员具体说明，毕竟告警只能起到辅助作用，决策者是飞行员，不了解原理很难做出正确的判断。4、处置程序中应尽量遍历所有可能，本事故中飞行员打开左右油箱交换阀后没有及时关闭也是造成燃油全失的一个重要因素。在手册中应明确各按键开关开启和关闭的时机和条件。S1-8：华航006号-747-1985年:747共配备4个引擎，其中4号引擎由于节流阀发生了千分之四的磨损造成供油量不足，无法提供足够推力，机长在过高高度错误重启，使该引擎彻底熄火。左右侧推力的不平衡使飞机开始向右滚转，飞行员没有退出自动驾驶接管飞机控制，同时该机型的自动驾驶仪又没有方向舵的控制功能，造成飞机失控反转加速俯冲。启示：1、维护工作一定要精益求精，千分之四的磨损都有可能造成不可挽回的灾难。2、自动驾驶功能的设计一定要完备，不能像本次事故一样，自动驾驶仪竟然没有对于方向舵的控制。同时，对什么情况自动驾驶不建议接入要向飞行员做出明确的说明。在设计之初，就要对不能接入自驾的场景进行约束，设置合理的接通退出条件。比如在单发失效的情况下，自动驾驶仪就应该自动退出。3、对飞行员飞行手册的培训一定要从原理入手，不能只讲现象。比如本次事故中，水平仪明明是机械仪器，但飞行员却认为是电子部件，在正常工作的状态时认为其发生了故障。飞行员在低温氧气稀薄的高空重启发动机，也明显是因为并不清楚检查程序中为什么要规定必须在低空才能重启。4、对于在什么情况下显示器仪表板上的数据不可信一定要在手册中给飞行员以明确的说明。以免飞行员因对数据的怀疑贻误操作时机。

调查出来的空难原因：飞机硬件(大多数因为引擎坏了，基本都是引擎爆炸着火)或者电脑系统突然故障，空管指挥不当，地勤修理不当，机组人员疲劳驾驶或飞行员错误操作等飞行技术问题，鸟突然出现，地图不对，极端天气如暴风雨冰雹，货仓中锂电池着火，恐怖分子劫机或有人想自杀，checklist太长机组没时间check完，机组到死都不知道什么原因导致飞机失控而乘客知道原因却无法与机组交流……

新知识：

1.飞机爆炸着火后的烟是有毒的，很多人是被烟雾毒死的而不是被烧死

2.坐在飞机靠后的位置存活机率更大？？？

3.海上迫降不成功的话全靠自己的游泳实力了 4.空难大多数是因为人为因素

2.要学会游泳

3.一定要好好安检！

【4星】 1、6集繁体字幕，看了前三集。第二集QQ禁传，不知道是又涉及了他们的什么G点。 2、拍得挺好的，前面首先介绍一下最后的大致结果，然后通过情景复现，让观众身临其境感受到危险，或许有挑火的嫌疑，但还是能引起人们的警醒 3、中间部分让人紧张，有点意思，所以虽然飞机✈的失事率低，但感觉还是不如高铁安全 4、很多空难都不是一下子发生的，都是逐渐发生的，而最后之所以酿成大祸，都是主客观、内外部条件凑一起了，这就像指数函数一样，爆炸性增长。本来外部环境——天气、飞机有瑕疵的设计就不行，再加上驾驶员的紧张、被航空公司的条款指引向错误的方向，如此种种最终导致了飞机失事 5、以前感觉只要是空难一般都会全死，看了几个下来发现，原来空难也不一定就乘客全没，也会有幸存者。当然，没有幸存者也是常见的
